Single-Question Prescreens

These questions are used to prescreen patients to determine if any full screenings (AUDIT, DAST, PHQ9) are necessary.
1. How many times in the PAST 12 MONTHS have you had four (five for men) or more drinks in a day?


2. How many times in the PAST 12 MONTHS have you used marijuana, an illegal drug, or a prescription drug for nonmedical reasons?


3. Over the PAST 2 WEEKS, have you often been bothered by...
a. Little interest or pleasure in doing things?


b. Feeling down, depressed, or hopeless?
